Insurance & Fees

Justin Tobin is an out-of-network provider with all insurance plans. Therapy session fees are as follows:
Individual Counseling Initial Appointment: $275
Individual Counseling, 55 minutes, on going: $250
Couples Counseling, Initial Appointment $350
Couples Counseling, 55 minutes, on-going: $300
Family Counseling: inquire as this depends on number of family members.

Questions to Ask Your Insurance Provider

• Do I have out-of-network benefits for mental health therapy?
• What is my co-pay or co-insurance for each out-of-network session?
• Do I have an out-of-network deductible?
• What is the total deductible amount?
• How much of it has been met this calendar year?
• Are the following CPT codes covered under my plan?

Superbill/Receipt

As an out of network provider, I do not submit claims to your insurance company. After each appointment, you will receive a detailed receipt (a.k.a. superbill) that includes all of the required information for you to receive reimbursement, per your out of network benefits. Most insurance plans offer an online portal for submitting receipts and claims; however, please verify this with your provider.
